Default Shaking & Panting at night

My 12 yr. old Yorkie shakes and pants at night and will not let me hold him or comfort him anymore. During the day he sleeps in the closet or walks around with his tail between his legs. Eager eater, but is losing his sight and hearing.
I left him for a month with a good friend, and even though I've been home 3 months he won't have anything to do with me, except when I take him for a walk or feed him. He coughs a lot and has elevated liver enzymes. Has had Pancreatitis and used to have seizures infrequently. A "dog psychologist" said he needs pet training to heel, etc. but I disagree. Is it dementia? or is he still mad at me? Has anyone experienced this? Veterinarian is no help.

It may be dementia.

At the same time, panting, shaking and the tail between the legs are signs of pain.

What are you doing for the liver and pancreas?
